"Continuum of Care is a small network of houses which are used as a staging ground for rehabilitation and an alternative location for readjustment of clients, who went to the hospital for various reasons. The purpose of the houses is to provide an alternative place to the hospital, which Continuum provides by directly addressing the endemic problems that sent the individual to the hospital in the first place. The goal is to provide a calmer and more client centric care system, which is something that Continuum excels at."
