"Very disorganized environment, does not have established policies and practices, uses code review as a means of disciplining employees, coding begins before requirements are complete yielding ever-shifting work and rework, everyone seems to put in 12 hour days, but no work ever gets done."

What suggestions do you have for management?

"Start by getting rid of all the developers, architects, and managers, switch to C# so you can find some people who know software development, and build the applications from scratch."
