"The lack of emphasis on Quality that eventually crept into company operations. The total lack of independent oversight afforded by an strong, executive-level supported Quality Management Division."

What suggestions do you have for management?

"Rebuild the Quality Management Division, and make the head of this organization at least a Sr. Director reporting to the company COO or CEO. Ensure QM independence and executive-level support. That said, QM must work with Operations and Engineering to streamline processes and procedures to ensure quality service while remaining responsive to cost and schedule."
