Job Overview:

As a PRN Occupational Therapist, you will help patients regain independence and improve their quality of life by providing individualized therapy in the comfort of their homes. This role offers the opportunity to develop meaningful patient relationships, collaborate with a supportive team, and work with flexibility—all while making a significant impact on patient outcomes.

Key Responsibilities:

* Conduct evaluations to assess functional status and suitability for in-home therapy.
* Design and implement personalized care plans that align with patient goals.
* Communicate with interdisciplinary care teams, physicians, and family members.
* Provide skilled occupational therapy interventions to promote independence and safety.
* Document care and progress accurately using an electronic health record system.
* Contribute to a positive team culture built on compassion, accountability, and collaboration.

Requirements:

* Current licensure as an Occupational Therapist in the state of practice.
* Valid driver’s license and proof of auto liability insurance.
* Minimum one year of experience in acute care, rehabilitation, or home health preferred.
* Strong communication, critical thinking, and time management skills.
* Comfortable with mobile technology and digital documentation systems (HCHB experience a plus).
