Physician Assistant – Community Health

Annual Salary of around $130k-$150k- varies depending on years of experience!

Position Summary

They’re seeking a compassionate and experienced Physician Assistant to join their primary care team. This provider will work in a collaborative, team-based environment offering comprehensive outpatient care to pediatric, adult, and geriatric patients. The role is ideal for a motivated clinician who thrives in a community-focused setting and is committed to health equity.

Key Responsibilities

* Deliver primary care services including exams, diagnosis, treatment, follow-up, and health education
* Manage acute and chronic conditions across all age groups
* Collaborate closely with physicians, nurses, and behavioral health providers to ensure coordinated, holistic care
* Participate in quality improvement initiatives and clinical case reviews
* Maintain accurate and timely medical documentation in the electronic health record system
* Engage patients in preventive care planning and health promotion strategies
* Contribute to a culture of teamwork, innovation, and accountability within the clinic

Qualifications

* Graduate of an accredited Physician Assistant program
* Current PA license in New York
* Minimum of one year of clinical experience in an outpatient or primary care setting
* Comfort treating patients across the lifespan: pediatrics to geriatrics
* Strong interpersonal and communication skills, with cultural humility and a community-first mindset
* Bilingual preferred, but not required
