JOB SUMMARY
Transitions of Care - Clinical Pharmacist II
Primary duties will be patient education on high risk patients. Determining medication barriers and access barriers. Working with the inpatient pharmacists, case managers, and providers to ensure patient education, proper discharge medication regimen. Providing data to leadership demonstrating the effectiveness of a pharmacist led transitions of care program.
REQUIREMENTS
- Required Bachelor's Degree Pharmacy
- Preferred Doctorate Degree Doctor of Pharmacy (Pharm.D.)
- Completion of a PGY-1 Pharmacy Practice Residency or a minimum of three (3) years of direct patient care experience in hospital setting
- RX - Pharmacist
- BLS CPR - Basic Life Support
