Job Requirements are:

Education: Bachelor's Degree from an accredited college or university in a social service-related field.

Licensure/Certification: Valid driver’s license and auto insurance required. License by the West Virginia Board of Social Work Examiners or equivalent board preferred but not required.

Experience: Minimum two years' experience in children and family services, with preferable knowledge and demonstrated experience of trauma. A master's degree in a related Social Services field may suffice for lack of experience.

POSITION SUMMARY The Care Manager will be responsible for attending meetings and being a part of the treatment team for foster and adoptive families, developing TLVY youth plan surrounding the development of life skills and preparation for independent and self-sufficient living, overseeing/consulting on PBIS instruction and framework.

Staff members are expected to work with children and families, community partners and each other using KVC values of Respect, Accountability, Integrity, Open Honest Communication, Teamwork and Continuous Improvement. Additionally, staff members are expected to be role models for children and families, be solutions focused and build upon strengths. KVC policies employees are also expected to maintain knowledge of and adhere to all company policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES Other duties may be assigned according to qualification of staff and development of programs.

• Continues to build welfare systems that are safe, secure, and stable; family centered, connected to the community; and which relies on evidence-based practice necessary to support contract services.

• Will act as a member of a treatment team and give feedback on youth progress.

• Will attend all Multi-Disciplinary Team (MDT) meetings and IEP meetings.

• Will travel to multiple counties in the service area and be able to work in the homes of the families that are served.

• Will be knowledgeable in the areas of child development, parenting skills and trauma.

• Will ensure that TLVY standards are met through documentation and keeping the youths chart up to date. • Have knowledge of and provide de-escalation techniques, crisis management support, and Positive Behavior Intervention System (PBIS) directives. •

Will participate in a 24 hour on call rotation.

• Will work with community members involved in the youth’s life including the birth family or birth extended family.

• Will provide supervised visitation and will document the outcome of the visits. • Will have a good working relationship with the Department of Human Services.

• Meets with Program Manager for a minimum of one hour bi-weekly for supervision.

• Ensures that all assessments and documentation are up to date for youth. This includes Ansel Casey and Transition Plan for youth aging out of care.

• Will have knowledge of Medically and Socially Necessary service techniques and proper documentation of these services.

• Ensures that intake paperwork is completed in a timely manner. Completes progress summaries and Q/A of charts.

• Responsible for meeting performance expectations including contract requirements, timelines, regulations, and laws.

• Provides direct services per program, state, and contract guidelines. • Ensures services follow all regulatory standards and contract terms and conditions.

• Completes all required paperwork and follows agency policies and procedures within timelines, to ensure smooth paper flow.

• Attends and participates in all scheduled meetings, court hearings, consultations, and staff meetings, as required.

• Acquires and develops resources to meet identified needs of youth and staff.

• Provides positive feedback to youth and staff.

• Provides culturally relevant and sensitive services to clients and ensures staff do the same.

• Provides back-up support to contract services as requested.

• Observes children for signs of abuse and/or neglect.

• Maintains working relationship and communications with DoHS, FSU, DTP 2 REV 4/2/2025

• Runs and organizes planning team meetings for youth

• Understands contract requirements and works to improve contract specific outcomes.

• Attends meetings, seminars, and other educational functions as identified by the supervisor or agency director.

• Completes ADP Learning and annual courses according to agency schedule. • Has a valid driver’s license and auto insurance.
