POSITION SUMMARY
Goshen Community Services of Nebraska is seeking a compassionate and experienced Registered Nurse (RN) to support individuals with developmental disabilities through the Nebraska Medicaid Waiver (DD). The Nurse will ensure medical care aligns with individual support plans, provide direct care and oversight, coordinate health services, and train staff. KEY RESPONSIBILITIES Health Assessment and Monitoring
- Conduct initial and ongoing health assessments for individuals served by the agency.
- Monitor individuals' medical conditions and provide recommendations to ensure optimal health outcomes.
Medication Management
- Oversee medication administration and storage, ensuring compliance with state and agency policies.
- Train staff on proper medication administration techniques and documentation.
Training and Development
- Provide training to staff on medical protocols, including safety and infection control.
- Educate staff on recognizing and responding to health concerns or emergencies.
- Provide Med Aid training to staff.
Collaboration and Communication
- Work closely with Program Coordinators, Direct Support Professionals, and families to coordinate care.
- Serve as a liaison with physicians, therapists, and other healthcare providers.
- Attend Individualized Support Plan (ISP) meetings to provide input on health-related goals and objectives.
Documentation and Compliance
- Maintain accurate and up-to-date medical records for all individuals served.
- Ensure compliance with all state, federal, and agency policies regarding healthcare and medication.
- Assist in the preparation and submission of required medical documentation to governing agencies.
Emergency Response
- Provide immediate medical care in case of emergencies.
- Develop and implement emergency health care plans for individuals as needed.
SKILLS & QUALIFICATIONS Education:
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the State of Nebraska.
- Bachelors Degree or a minimum of 5 years experience in a related field.
Experience:
- At least 1-3 years of nursing experience, preferably working with individuals with developmental disabilities or in a community-based setting.
- Experience with DD Waiver services strongly preferred.
Requirements:
- Valid RN license in good standing in Nebraska.
- Valid drivers license and vehicle insurance.
- Pass State and National background checks (including APS/CPS).
- Must pass a physical, drug screen, and be able to lift up to 70 lbs.
ESSENTIAL FUNCTIONS
- Conduct routine assessments; seek medical attention for members when needed.
- Coordinate and attend medical appointments if warranted.
- Liaise with physicians, guardians, staff, and external providers.
- Perform on-site visits and monitor health-related documentation.
- Provide medical training to team members.
- Attend client team meetings and ensure medication compliance.
- Respond to emergencies and provide direct care if needed.
- Ensure compliance with all state/federal nursing regulations and individual Service Plans.
- Monitor and document adverse medical events and follow up accordingly.
- Manage MARs and assist in all aspects of medication administration and tracking.
- Provide oversight for adaptive equipment and psychotropic medication use.
- Report communicable diseases and ensure treatment coordination.
- Maintain confidentiality of all resident and medical information.
- Other duties as assigned.
ADDITIONAL RESPONSIBILITIES Intake Meeting Responsibilities:
- Obtain and verify medication lists and pharmacy information
- Coordinate with the Coordinator of Services to ensure a side effect tracker is created
- Confirm program tracking for Schedule II and psychotropic medications
Medication Oversight:
- Ensure noon medications are securely stored in a locked cabinet at Day Services
- Oversee proper administration and documentation of all noon meds
- Review Due Medications report daily to ensure no missed doses
- Follow up with Residential Manager regarding medical appointments and consult documentation
Due Dates & Routine Reviews:
- Daily: Review MARs
- Weekly: Check Therap To Do list for new clients
- Monthly: Conduct walkthroughs Section 1: Medication Reviews
