Program OverviewFamily Centers Healthcare seeks a full-time experienced, energetic, and compassionate Advanced Practice Nurse Practitioner (FNP or PNP) or Physician Assistant (Primary Care) to work as part of a dynamic, multi-disciplinary team to deliver comprehensive primary medical care to patients. This opportunity would combined primary care skills in both an outpatient setting and within a school setting in a School Based Health Center.Family Centers Healthcare is a Federally Qualified Health Clinic (FQHC) located in Greenwich and Stamford. The clinic provides primary healthcare, dental and mental health services to those living in Greenwich and Stamford communities and the surrounding neighborhoods.The RoleIncumbent will be responsible for the diagnosis and treatment of common and chronic illnesses, planning of periodic health assessments, vaccines, screenings, promoting optimal health and preventative care. Additional responsibilities include community referrals, accurate and comprehensive documentation, providing patient education, and other regulatory compliance and training requirements.Qualifications And Educational RequirementsMaster’s or post-Master’s degree from an accredited program and at least 2 years of relevant experience Demonstrated aptitude in providing medical services largely geared to under-served populations.Current active license to practice in the State of ConnecticutMedical malpractice insurance (in addition to that offered by the agency)Current active DEA licenseAbility to work flexible schedule including day and early evening hoursBilingual - Spanish preferredAbout Family CentersFamily Centers is a private, nonprofit organization offering health, education and human service program to children, adults and families in Fairfield County.
