Job Description For (LPN)

·        Supervise day-to-day function of Nurse Assistants in accordance with current rules, regulations and guidelines governing long-term care facilities  

·        Ensure that written policies and procedures governing day-to-day functions of the Nursing Department are followed by staff.

·        Ensure that the Nursing Policy and Procedures are followed in rendering nursing care.

·        Cooperate with other resident services when coordinating nursing services to ensure that resident’s total regimen of care is maintained.

·        Ensure that all personnel follow respective job descriptions.

·        Perform administrative duties such as completing medical forms, reports, evaluations, studies charting, etc., as necessary.

·        Participate in the development, maintenance, and implementation of Paramount Community Living and Rehab Quality Assurance program for the Nursing Department.

·        Update plans of care as required.

·        Assist in planning nursing services' portion of resident plans of care as necessary.

·        Participate in surveys made by authorized government agencies.

·        Make written and oral reports/recommendations concerning shift activities as required.

·        Periodically review department policies, procedure manuals, job descriptions, etc.  Make recommendations for revisions.

·        Meet with assigned nursing staff, as well as support personnel, in planning the shifts' services, programs and activities.

·        Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required.

·        Admit, transfer and discharge residents as required.

·        House Supervisor responsibilities may be assigned as needed.

·        Perform other related work as directed by your supervisor.

·        Fill out and complete accident/incident reports.  Submit to your supervisor as required.

·        Chart all resident accidents/incidents.  Follow established procedures.

·        Encourage attending physicians to review treatment plans, record and sign orders, progress notes, etc., in accordance with established policies.

·        Chart nurses notes in an informative and descriptive manner that reflect resident care, as well as the resident response to the care.

·        Complete and file required record-keeping forms/charts upon resident admissions, transfer and/or discharge.

·        Receive telephone orders from physicians and record on Physicians' Order Form / Telephone Order Form.

·        Record new/changed diet orders.  Forward information to the Dietary Services Department.

·        Transcribe physician's orders to resident charts, medication administration and treatment administration records and care plans.

·        Clarify any discrepancies noted concerning Physician Orders or diet changes with physician.  Report documentation errors to your supervisor. 

·        Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.

·        Sign and date all entries made on residents' medical records.

·        Complete Resident Change of Status Form in accordance with established procedures, making sure form is complete.

·        Complete all nursing assignment paperwork completely and correctly.
