As part of the Clinical Professional Development and South Shore Hospital Wound center, the Wound, Ostomy, Foot Care nurse supports the Wound, Ostomy Program Manager in providing skilled nursing care of patients with stomas, draining wounds, fistulas, arterial, venous, traumatic, malignant, diabetic and neuropathic wounds, pressure injuries, and MASD (Moisture Associated Skin Damage). Provides consultation and teaching of specialized patient care needs to medical, surgical, and nursing staff, patients, families, and other community members and works well with all members of the hospital community. Provides daily support for the Podiatry and Lower Limb Preservation (LLP) teams including procedure assist, inpatient clinical documentation, patient and family education including coordination of follow up outpatient care, and bedside wound, skin and foot care including nail debridement. Collaborates with the Wound Center, Home Care Division and Product Representatives and assists the WOCN Program Manager with orientation and education programs for clinical colleagues.

With the WOCN Program Manager, assesses designs, presents and coordinates and evaluates educational programs. Assists with in-service and orientation for designated nursing division, for nursing and non-nursing personnel, to ensure competence while advancing the knowledge, skills, and ability at all provider staff levels.

ESSENTIAL FUNCTIONS

1 - Provides direct care to ostomy/skin integrity impaired patients that is based on current scientific principles and professional practice models. Supports the Wound Ostomy Service every Monday; additionally, will support the Wound Ostomy service Tuesday through Friday based on availability.

a - Performs initial assessment for consults and re-assesses as necessary or whenever the patient's condition changes or deteriorates.

b - Implements and evaluates the plan of care at regular intervals to ensure adherence to quality assurance standards and optimum patient outcomes

c - Identify high-risk settings and groups to target prevention efforts to minimize risk.

d - Assesses individual risk of developing pressure ulcers based on utilizing the Braden scale and through the use of wound photography.

e - Promotes patient independence and confidence in self-care through consultation and skilled nursing interventions.

f - Serves as a resource to hospital case managers, nursing staff and physicians with discharge planning and outpatient referrals, including referrals to Wound Center and home with services.

g - Evaluates, selects, and recommends supplies and equipment for patients with ostomies, draining wounds, fistulas, pressure sores, and incontinence, foot and nail disorders.

2 - Supports podiatry and LLP teams Tuesday through Friday providing direct care to patients with lower limb, foot and nail disorders

a - Assists on-call and other in-hospital DPMs with O.R., E.R., and bedside foot and nail procedures

b - Participates in daily rounds coordinating patient's treatment plans with the podiatry and LLP team members as well as admitting service and other specialty consults.

C - Coordinates transition of care with VNA/Rehab facility upon patient's discharge ensuring appropriate wound orders and referrals are in place.

d. - Provide patient/family education regarding wound management, durable medical equipment, indications to seek care after discharge for urgent or emergent concerns.

e. - Perform foot care in accordance with the Massachusetts Board of Registration in Nursing, M.G.L., Advisory Ruling Number 9305.

3 - Updates the Plan of Care, teaching plan, discharge plan and outcome criteria are reassessed to meet client and family/caregiver needs and to assure quality of care, as evidenced by documentation in the clinical record.

a - Coordinates and communicates with other clinical team members, ensuring that all members of the healthcare team are aware of medical or physical conditions resulting in a change in the plan of care.

b - Initiates and/or participates in the client care conferences, as needed.

c - During in-patient visits, identifies need for various services and makes appropriate internal and community referrals, as evidenced by documentation in the clinical record.

d - Provides consultation to clients, nursing staff, physicians, and other healthcare team members related to area of expertise.

4 - COMMUNICATION

a - Communication mechanisms to patient are timely, accurate, and thorough.

b - Communication mechanisms to physicians are timely, accurate and thorough.

c - Communication mechanisms to other members of the healthcare team are timely, accurate and thorough.

5 - DOCUMENTATION - Maintains up-to-date documentation of nursing care provided to assure the integration of information for use by the healthcare team, in order to assure quality care.

a - Medical Records are well maintained and secured

b - Documentation supports coding pertinent to each visit

c - Referrals are made based on identified needs and communicated to the healthcare team according to policy

d - Practices within the legal boundaries of nurse practice act and according to Hospital policy and procedure.

6- CONSULTANT/CLINICAL EXPERT

a - Participates in the clinical supervision/evaluation of designated staff members.

b - In collaboration with the WOCN Program Manager, Nurse Manager and Director, oversees and monitors the effectiveness of care at the unit level.

c - Serves as a clinical and instructional role model by providing direct patient care as needed or requested.

d - Acts as an expert consultant to clinical nurses, APCs, and physicians to optimize the effectiveness of care in achieving desired patient outcomes.

e - Assimilates the findings of nursing research and evidence-based practice into nursing educational programs and clinical practice.

f - Assists in the development of hospital and nursing policies, procedures, guidelines and order sets specific to practice specialty.

g - Identifies ethical issues in nursing practice and guides staff and other providers in problem resolution by facilitating direct resolution or engaging the Ethics Consultation Service.

h - Identifies and meets nursing care needs of patients and their families integrating knowledge of the disease process into clinical practice.

7 - ORIENTATION

a - Collaborates with the WOCN Program Manager and Nurse Manager to develop individualized orientation programs for all new staff members within the department

b - Participates in the Nursing Department central orientation programs.

c - Maintains competency based orientation checklists for all staff within the department.

d - Collaborates with and mentors unit based preceptors to provide comprehensive orientation experiences for all new staff members.

e - With the WOCN Program Manager, organizes combined meetings with the orientees, preceptors and the nurse manager to review orientation processes, provide timely feedback to the new staff member and address opportunities for improvement.

f - Provides consultation services to Managers relative to learning and practice issues including the development of staff member specific learning management plans.

8 - EDUCATION

a - Collaborated on an annual educational needs assessment for the division of nursing

b - Coordinates and assists with the presentation of mandatory education as evidenced by staff member completion percentages.

c - Develops unit based/service based educational programs based on the annual needs assessments.

d - Responds to and maintains a flexible approach to newly identified educational needs.

e - Coordinates with the WOCN Program Manager on new product, policy and procedure in-services on all shifts.

f - Provides continuing education programming for staff within the unit, department and organization.

9 - COMMUNITY INITIATIVES - Develops, implements, coordinates community health related projects.

a - Participates in outreach activities to further market service line specific services and programs.

b - Promotes collaboration between SSH, The wound Center, podiatry, LLP, and the VNA/Hospice to support quality care across the continuum

10 - TECHNOLOGY - Embraces technological solutions to work processes and practices.

a - Demonstrates proficiency in the use of all Epic applications used in clinical areas

b - Uses web-based incident reporting system when appropriate.

c - Able to access and manipulate clocking and calendar options within Time PC application

d - Expertly uses and teaches appropriate use of biomedical equipment used in the assigned clinical area.

JOB REQUIREMENTS

Minimum Education - Preferred

Bachelor's Degree in Nursing

Minimum Work Experience

Three to five (3-5) years of progressively responsible nursing experience relative to the designated clinical services preferred.

All American Nurses Credentialing Center Nurse planners are required to hold a graduate degree AND his/her baccalaureate or the graduate degree must be in nursing.
