Clinical areas of coverage include:Hospice patientsConfluence Health discharge patients, including Day Surgery and the Emergency Department.Confluence Health clinic patientsPatients covered by the Compassionate Care Program The position is responsible for therapeutic assessment, interventions and plans for individual patients as well as patient populations. For Hospice patients, the Pharmacist assumes prescriptive authority, managing the daily comfort needs and terminal pain control. The Pharmacist works closely with the Hospice nurses who report patient status. Documentation for the Hospice patient encounters are documented in both the Hospice EHR (HomeCare HomeBase) and the Pharmacy EHR (Epic Hyperspace and Willow Ambulatory). The Pharmacist is a valued member of the Interdisciplinary Care Team and assumes responsibility for medication related outcomes. Position Reports To: Outpatient/Hospice Pharmacy Manager Essential FunctionsEnsure safe, effective and cost-effective use of drugs on behalf of the patient through the application of specialized knowledge, skills and professional judgement, while considering the patient’s individual condition.Maintains knowledge of age specific issues and applies that knowledge to the provision of care within our institution.Ensure compliance with hospital and departmental policies, procedures and practice guidelines.Provides audience appropriate drug information to patients, nurses and providers.Monitors medication therapy within the scope of practice as outlined in State law.In collaboration with the Hospice physicians and nurses, performs ongoing assessment of patient risk factors, indications for therapy and works for optimization of therapeutic regimens, adjusting medication therapy as described in protocol and as authorized by CDTA.Documents appropriately in the Hospice patient electronic medical record(s) including communication with members of the patient care team.Demonstrates competency and proficiency in essential job functions by maintaining active credentialed status via Confluence Health delegated credentialing through the Medical Staffing Office.Supports the continuum of patient care through communication and coordination of home IV drug therapy services with the Home Infusion Pharmacist.Participates in the weekly Hospice IDG meeting at least once per month.Provides direct supervision to pharmacy technicians and assistants. Responsible for overseeing and monitoring the daily activities of pharmacy technicians and assistants to appropriately maintain the provision of safe, timely and effective pharmaceuticals.Commits to instructing, modeling and coaching pharmacists, technicians, assistants, students, residents and other health care disciplines as deemed appropriately by the Pharmacy Director.Assist in Adverse Drug Reaction and Medication Error reporting and contributes ideas for prevention of further incidents.Maintain patient confidentiality standards.Maintain active Basic Life Support (CPR) certification.Maintain DEA license.Share the standby/on call hours with the other Outpatient Hospice Pharmacists.Coordination of the drug distribution system/pharmacy services for areas of responsibility.Ensure on-going improvements in therapy and systems through collaborative problem solving, research and participation in the hospital and department’s performance initiatives.The position has independent responsibilities for organizing and planning ones work in order to accomplish daily responsibilities as well as broad goals and objectives.Other duties as assigned. Qualifications Required:Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy degree from an accredited school of pharmacy.Licensed to practice pharmacy in the state of Washington prior to start date.Strong computer skills, ability to work in multiple systems at the same time.Certification: Hospice and Palliative CareMust achieve within 1 year. Desired:PGY1 residency training and/or the equivalent of three years as a licensed pharmacist in the anticipated practice area.Bilingual Spanish/English with ability to pass a fluency evaluation from a CH chosen vendor.Strong communication and leadership skills.Prior experience in Hospice pain management.
