The Nurse Manager manages the provision of nursing care to patients, evaluate nursing practice and provide clinical learning activities for staff. The Clinical Head Nurse is a registered Professional Nurse responsible and accountable for the delivery of patient care in an outpatient setting.

Utilizes the Nursing Process in evaluating and assessing the Nursing needs of patients and managing patient flow in the HOPE Center. Manages & administers the provision and administers Nursing Care to patients to insure adherence to standards of unit and department along with all applicable local, state and federal guidelines.

Responsible for overall operation of the HOPE’s medical and nursing program including:

a. Ongoing, proactive, and effective communication with clients, staff, and other Departments in hospital to assure the highest level of client/patient care.

b. Maintaining positive working relationships with coordinating and referring/referral agencies and program staff to assure positive program outcomes.

c. Performs management activities including interviewing, hiring, evaluation, and termination of Nursing staff (RN and Medical Assistant level) and assigned grant funded program(s). This also includes scheduling staff time off and ensuring adequate staffing coverage to provide safe and effective client care.

d. Direct and supervise the staff promptly addressing all program challenges in a pro-active and solution-oriented manner.

e. Provides clinical learning activities for all staff.

f. Cover Nursing duties, including phlebotomy, to ensure there is no interruption of patient care.


Assure fiscal viability of the HOPE Center by implementing program changes to assure that services continue to be billable (and paid) by managed care, insurances and grantors. Supervises the Hepatitis C grant funded program (and other programs, as assigned) to assure that all grant deliverables are met and that high quality patient care is performed. Evaluates and improves overall nursing practice at HOPE Center through participation in Department’s Quality Improvement and Patient Center Medical Home efforts.


This includes participation in yearly performance improvement activities for programs supervised by this position. Manages Departmental client referral system ensuring maximized reimbursement of delivered services. This will include interfacing with insurance companies to obtain prior authorizations for patient medications and procedures as ordered by providers. Identifies goals for professional self-development and professional development of staff and seeks continuing education opportunities to attain these goals. Identifies and implements goals for staff development and manages opportunities to attain these goals.


Works as member of Department’s management team to identify and implement cost savings and/or income enhancement within the service. Works cooperatively with the supervisor of the Care Management team(s) to assure that staff understands, and address aspects of the medical and nursing process related to effective care management of HIV+, Health Home, PrEP and clients. Works cooperatively with the Clinical Supervisors to assure that effective and efficient client care is delivered throughout the Department in a way that supports positive outcomes and a low-conflict environment. Other duties and responsibilities as directed by supervisor.


Qualifications

• New York State Licensed Registered Nurse

• B.S.N required, M.S.N. preferred.

• Demonstrated competence in medical outpatient Primary Care settings (2-3 years clinical experience) with in-depth current knowledge in areas of clinical practice.

• Ability to problem solve, communicate effectively, and organize resources appropriately.

• Demonstrated commitment to continued professional development.

• Possesses well-developed and effective interpersonal skills and is able to communicate effectively verbally and in writing.

• Demonstrates confidence in actions and exercises good judgment.

• Displays leadership ability and the willingness to assume authority and accountability for the direction and supervision of others.

• Possesses the ability to plan, organize, develop and implement goals, objectives, policies and procedures necessary for quality care.

• Embraces and adapts to change.

• Demonstrates ability to recognize problems, approach them in an objective manner, reach appropriate solutions, implement them and evaluate for effectiveness.

Preferred:

• Working knowledge of HIV-specific Performance Indicators

• Bilingual: Spanish/English is preferred.

• Experience treating individuals living with HIV/AIDS
