Community-Based Perinatal Patient Navigator

The goal of the Community Based Perinatal Patient Navigator (CB-PPN) is to navigate birthing individuals through Georgia's healthcare system. A Patient Navigator's role is to help remove any barriers to care the patient may encounter by identifying critical resources for patients, helping them navigate through the healthcare system, and promoting positive health outcomes for patients. The CB-PPN will engage patients, create a trusting relationship, navigate patients to appointments, and closely communicate with a patient's entire care team.

Minimum Qualifications

Education: High School Diploma or equivalent required

Experience:Exposure to issues of maternal morbidity and mortality

Participant of the CB-PPN Cohort

Preferred Qualifications

  • Ability to act as a bridge and educator between patients and their healthcare teams
  • Strong customer or patient service skills
  • Knowledge of maternal health issues in Georgia
  • Ability to provide Culturally Based Communication and Care
  • Detail-oriented with the ability to multitask
  • Proficient in all Microsoft Applications, including MS Office and Excel
  • Strong time management, organizational, and planning skills
  • Ability to work evenings and weekends, if needed

Immunization

Immunization Requirements

It is MSM's Immunization policy that all Prospective Employees are required to provide proof that they are vaccinated against COVID-19 prior to commencement of employment. If employment will commence during Flu Season, MSM requires all individuals to provide proof that they are vaccinated against Seasonal Influenza for the current Flu Season, unless granted an exemption. Failure to provide proof of vaccination for any required vaccines or obtain an exemption from MSM will result in rescission of a candidate's offer of employment or disciplinary action up to and including termination.
