Job Details
RN Hospice Case Manager (Polish speaking a HUGE plus)
Permanent Position
Position Details:
- Monday to Friday schedule, 8:30a - 5p
- Salaried position
- Mileage reimbursement
- Case load of 12 - 14 patients
- Will work both in homes and hospice facilities
- Charting is done on phone
- Huge value on work / life balance
- Freedom to build your day based on the cases in your schedule
Position Requirements:
- Previous RN experience (Hospice most ideal, Critical Care experience has been a strong transition into Hospice as well)
- Reliable vehicle / clean driver's license
- IL licensed
- Caring demeanor with a passion for Hospice care
