GLFHC is currently seeking a High Risk Obstetric (HROB) Nurse Care Coordinator. The HROB Nurse Care Coordinator serves as an integral member of the healthcare team and is responsible for coordinating the care of high risk OB patients from initial entry into care through the first year of the infant's life including postpartum visits, to ensure the optimum health of these high risk patients. The role promotes continuity of care for OB patients and newborns, and serves as a resource to clinicians and other clinical staff involved in prenatal care. The role will focus on facilitating timely entry to prenatal care, and identifying and coordinating appropriate delivery of prenatal and newborn care as provided by the healthcare team, at the direction of the prenatal clinician. The role supports the scheduling and delivery of appropriate health care and community services during the pregnancy and post-partum periods, and initial well child visits for the newborn. This position reports to the Care Coordinator Nurse Manager (CCNM), and works closely and collaboratively with Clinical Care Nurse Coordinators (CCNCs), medical leadership, and nurses conducting prenatal intakes at all GLFHC sites.

  • Coordinates with prenatal clinician and care team to develop a prenatal care plan for HROB patients addressing all identified problems, and supports care team in carrying out, and updating quarterly, the care plan in a timely
  • Coordinates nursing staff at all sites to ensure that patients who begin their prenatal care at GLFHC receive an initial Nursing intake and education, appropriate initial labs, CSS intake, WIC referral, and initial provider visit in a timely
  • Contacts High Risk OB patients directly as needed to coordinate care, including postpartum care, reminder calls to patients when necessary, handling triage calls from High Risk OB patients, and follow up on test results as directed by the prenatal
  • Ensures the development and maintenance of, constructive relationships with prenatal clinicians and care team members, including staff from all interfacing departments (Medical Services, Nursing Dept., Community Support Services, Operations, Information Systems and other departments).
  • Facilitates the development and maintenance of a database/reporting capability on high risk OB patients and utilizes these tools to identify and address missed care/gaps in care, and determine if care coordination has improved patient

Qualifications

Experience:

  • Minimum of 2 years nursing experience, preferably in perinatal health services or labor and delivery.
  • Ability to think critically and prioritize and organize patient care, with attention to detail.
  • Ability to work independently, and in partnership with other care team members.
  • Proficient computer (e.g. Athena or other EMR, spreadsheet, word processing, database), analytical and organizational skills required.
  • Strong verbal and written skills required.
  • Familiarity with Latino population; Spanish fluency preferred.
  • Minimum of Bachelor's degree in nursing or related field required, or actively pursuing degree.
  • Current BLS for Healthcare Providers certification.

Education:

  • Graduate of an accredited professional nursing program with current Massachusetts Registered Nurse license required.
