Schedule: Mon-Fri 8am-4pm or 4 days a week, 8am-6pm; weekend hours on a rotational basis
Salary: $85,000 / year
We are seeking a new PACT Nurse for our Program for Assertive Treatment (PACT) teams to provide treatment to adults with persistent mental illness. The PACT Nurse will work on an interdiciplinary team of nurses, clinicians, and outreach workers to assist those in need in the community. The PACT Nurse will work under the supervision of the Lead Nurse to conduct psychiatric assessments in addition to physical health needs in order to provide holistic nursing services. The PACT Nurse will be responsible for making appropriate referrals to community physicians, providing management and administration of medication in addition to providing a range of treatment, rehabilitation, and support services.
Other responsibilities include but are not limited to:
- Providing case management for an assigned group of persons served, including coordinating and monitoring the activities of the individual treatment team
- Conducting a comprehensive assessment of psychiatric history (e.g., onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health
- Providing ongoing assessment, problem solving, side-by-side services, skill training, supervision, and environmental adaptations to assist persons served with activities of daily living

- Two years experience in a mental health setting.
